8/23/2023 EnchantaVaughn Approved Behavior: Client demonstrates excellent conduct in the home. Client spoke to QMHP and communicated healthy statements about being in session today. Behavior: The interventions were viewed as successful. The client communicated he was in a cheerful mood. Client was supported with adhering to the rules in the home. The client also assists his mom with anything she needs done around the home. Client was applauded for his teamwork skills and presenting excellent behaviors. Client was reported to be having a good evening by his mother. Client did not demonstrate any unsafe actions by trying to hit or attack others. Mom shared client has made some major improvements, but she wants him to put some effort into finding a hobby and being independent. Client responded with positive gestures and continued following along with each prompt. Intervention: QMHP applied positive self-talk strategies to help client think positively at all times and to talk about his actions and how they will make people feel. QMHP utilized active listening skills while client was verbalizing information about his day. QMHP explored the clientâs capability to cope with his stressors and to get back on track. QMHP assisted client with finding and utilizing necessary resources at home. QMHP advised client to make the right choices and to keep up the good work. QMHP informed dad that client can benefit from other services, thus probing them to utilize resources in the community that will help client reduce his aggressive behaviors. Intervention: QMHP went through program overview with family to reiterate key points and program objectives that would pertain to their specific situation. QMHP reviewed with mother the ability to limit the stress in the home and to utilize the reward system. QMHP utilized active listening skills while probing mother on the affect ways that she attempts to reduce client's negative behaviors. Mother was praised for her efforts to better herself and her living situation by making sure she provides client with everything he needs for school and keep client healthy and safe. QMHP encouraged mom to talk to client about the new changes in a less therapeutic setting to prepare him for transition and dealing with his thoughts and feelings. Response: The client communicated happy and optimistic expressions. Client expressed positive comments about his family, as demonstrated by his compliments. The client has not overreacted with anger to minor frustrations or irritants. Client responded in a pleasant manner acknowledging the positive reinforcement that the QMHP provided. Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD. Client was able to reevaluate himself and display a more positive attitude toward peers and authority. Client demonstrated expectations and responded well to feedback. Response: Client participated in reviewing key concepts. Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Client is observed correcting his mistakes and making good choices. Client utilized meditation and relaxation techniques. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Client was able to share his feelings. Client expressed that heâs trying his best to do the right thing. Client nodded in understanding and communicated that he was going to continue making good choices and show positivity around others. Client was acceptive of extra help and support.
